Facility Use Guidelines/Agreement

Administrative Manager
1. All requests by individuals or groups should be referred to the Property Committee for approval.
2. No request will be approved when in conflict with First Presbyterian Church of Burbank activities.
3. Requests should be submitted at least one month prior to date of event. When this is not possible, the Property Committee chair may grant approval at his/her discretion.
4. Requests from the following types of individuals will be considered favorably:
§ Other churches
§ Charitable groups
§ Outside weddings conducted by First Presbyterian ministers or ministers approved by Session.
§ Community service groups (Facility Use by: (Name of Organization, Contact Person and Contact Phone #.)
5. It is suggested that the following minimum donations to the church, except from church members, or church sponsored or supported groups, be accepted from outside individuals or groups for use of the church facilities:
a. Classroom—$50
b. Fireside Room—$100 ($175.00 with Small Kitchen)
c. Fellowship Hall—$200
d. Kitchen—$150
e. Sanctuary—$300
f. Kitchen (Large) - $150
g. Sanctuary—$300
- Parking Stipulations - WE WILL NEED PROOF OF INSURANCE AND LAY OUT FOR THE EVENT AND SET UP.
- Certificate of Insurance : First Presbyterian Church of Burbank - Fax #: 818-842-5134 – General Liability which includes: Limits: General Aggregrate Products at $2,000,000; Comp/OP Aggregate included; Personal & Adv Insurance included; each occurrence $1,000,000; Fire Damage (any one fire).

6. First Presbyterian Church requests that all groups using any part of these facilities pay a $75 Custodial fee for clean-up work after hours, as needed.
7. All outside doors are to remain secured once function starts. At the close of the function all Outside doors are to be checked and locked. Failure to secure facility will be cause for deposit forfeiture.
Release and Responsibility Agreement
We have read the Facilities Use Guidelines and agree to the terms contained herein.
Members of our group hereby release and waive all claims they might have against the First Presbyterian Church of Burbank, or any member thereof, for any and all injuries that might occur to them during the normal course of our occupancy of the Church as agreed upon, being assured that all proper means have been taken to insure the safety of participants.
We also agree to pay the cost of repairing any damage to the property or furnishings (replacement of same if necessary) incurred by our group during its occupancy of the facilities.
